smartsystem Update (Version 29)
New exposure scenarios, more flexible avatar dialogues, and a new PTSD module
A multi-story construction scaffold. A crane basket slowly moving away from the building while the view down remains completely unobstructed. This is exactly where one of the new scenarios in the Anxiety Module begins—part of Version 29 of the smartsystem, which expands standard exposure therapy where traditional approaches reach their limits and allows the immersive nature of virtual reality to deliver its full clinical value.

Fear of Heights Exposure: Construction Scaffold and Crane Ride
A new fear of heights scenario is now available in the Anxiety Module.
Step-by-step exposure on a construction scaffold
Patients move along a multi-story construction scaffold, gradually approaching the edge at different heights and looking down over the side. The intensity of the exposure can be adjusted with precision—from the lower levels to the greatest height.
Loss of control during the crane ride
As an extension of this scenario, patients can enter a crane basket that slowly moves away from the building. Suspended high above the ground, they experience increasing distance from the building while having no control over the movement. For many people with acrophobia, this perceived loss of control—the feeling of no longer being able to stabilize themselves—is a major trigger that is difficult to recreate in a controlled therapeutic setting.
Outdoor Addiction Exposure: Alcohol and Cannabis in Everyday Situations
The Addiction Module has also been expanded with a new outdoor scenario in which patients are exposed to alcohol and cannabis in a relaxed summer environment. The setting can be customized, allowing the exposure to take place either while standing in cool water or while sitting comfortably on a picnic blanket.
Alcohol
Drinks are placed on the table. In addition, a bicycle with a cooler box is available, allowing patients to retrieve cold beverages themselves—adding another realistic everyday trigger beyond simply seeing drinks on a table.
Cannabis
Pre-rolled joints are available on the table and can be consumed within the selected environment, whether in the water or on the picnic blanket.
This everyday context is intentional. In clinical practice, craving rarely develops in a treatment room. Instead, it typically emerges in relaxed, socially meaningful situations like these, making them far more representative of real-world relapse risks than static trigger images alone.
Text-to-Speech Manager: More Control Over Avatar Dialogues
The Text-to-Speech Manager now provides greater flexibility when preparing therapy sessions. Previously saved dialogue lines can be reordered, making it possible to structure complete conversations between patients and avatars before the session begins while still controlling them dynamically during treatment.
For therapists, this means conversations can be prepared in advance without having to improvise every interaction during the session, while remaining flexible enough to adapt to each patient’s responses. This significantly expands the possibilities for social exposure scenarios and role-playing exercises.
New Module: PTSD Following War-Related Trauma
A newly developed standalone PTSD module is now available for trauma resulting from war experiences. It has been designed for both civilians affected by war and military personnel with combat experience.
A detailed overview of the scenario design and current evidence is available in this article.
