Depression<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-8efd400 elementor-widget elementor-widget-text-editor\" data-id=\"8efd400\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>La d\u00e9pression est l\u2019un des troubles psychiques les plus fr\u00e9quents, associ\u00e9 \u00e0 une alt\u00e9ration importante du fonctionnement. Seuls environ 60 % des patient\u00b7e\u00b7s d\u00e9pressifs r\u00e9pondent \u00e0 un traitement pharmacologique ou psychologique, avec un taux de rechute \u00e9lev\u00e9.<sup>1<\/sup> <\/p><p>Chez les personnes souffrant de d\u00e9pression, le traitement de l\u2019information est perturb\u00e9 : elles dirigent pr\u00e9f\u00e9rentiellement leur attention vers des \u00e9v\u00e9nements n\u00e9gatifs et les m\u00e9morisent plus facilement. De plus, le traitement des \u00e9v\u00e9nements positifs est alt\u00e9r\u00e9 de mani\u00e8re n\u00e9gative. Ces biais cognitifs peuvent persister m\u00eame en r\u00e9mission et sont associ\u00e9s \u00e0 un risque accru de rechute.<sup>2<\/sup> <\/p><p>La pleine conscience implique de porter son attention sur le moment pr\u00e9sent (pens\u00e9es, \u00e9motions, perceptions corporelles, conscience et environnement) dans une attitude non jugeante.<sup>3<\/sup><\/p><p>Dans la litt\u00e9rature scientifique, l\u2019efficacit\u00e9 antid\u00e9pressive des interventions bas\u00e9es sur la pleine conscience (MBI \u2013 mindfulness-based interventions) a \u00e9t\u00e9 principalement d\u00e9montr\u00e9e dans le cadre du MBSR (mindfulness-based stress reduction) et du MBCT (mindfulness-based cognitive therapy).<sup>3<\/sup><\/p><p>Par ailleurs, des aspects de la pleine conscience sont int\u00e9gr\u00e9s dans d\u2019autres approches cliniques telles que l\u2019ACT (Acceptance and Commitment Therapy), qui contribuent \u00e0 am\u00e9liorer les sympt\u00f4mes d\u00e9pressifs ou \u00e0 r\u00e9duire le risque de rechute des \u00e9pisodes d\u00e9pressifs.<sup>3<\/sup><\/p><p>Les biais cognitifs sont trait\u00e9s \u00e0 l\u2019aide de programmes informatis\u00e9s appel\u00e9s programmes de Cognitive Bias Modification (CBM). Ceux-ci entra\u00eenent l\u2019attention vers des stimuli positifs et visent \u00e0 am\u00e9liorer leur traitement (entra\u00eenement \u00e0 la positivit\u00e9).<sup>2<\/sup>  <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b8ef45d elementor-widget elementor-widget-heading\" data-id=\"b8ef45d\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Achtsamkeit als therapeutischer Ansatz bei Depression<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-751b61b elementor-widget elementor-widget-text-editor\" data-id=\"751b61b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Achtsamkeit beinhaltet die Aufmerksamkeit auf den gegenw\u00e4rtigen Moment (Gedanken, Gef\u00fchle, K\u00f6rperwahrnehmungen, Bewusstsein und Umwelt) in einem nicht-bewertenden Sinne.<sup>3<\/sup><\/p><p>In der wissenschaftlichen Literatur ist die antidepressive Wirksamkeit von MBI (mindfulness-based interventions) vor allem im Rahmen der MBSR (mindfulness-based stress reduction) und MBCT (mindfulness-based cognitive therapy) gezeigt.<sup>3<\/sup><\/p><p>Zudem werden Achtsamkeitsaspekte in verschiedene andere klinische Ans\u00e4tze wie der ACT (Acceptance and Commitement Therapy) integriert, die zu einer Verbesserung der depressiven Symptomatik oder das R\u00fcckfallrisikos depressiver Episoden f\u00fchren.<sup>3<\/sup><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-dffb02b elementor-widget elementor-widget-heading\" data-id=\"dffb02b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Cognitive Bias Modification (CBM) bei Depression<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-8e099d0 elementor-widget elementor-widget-text-editor\" data-id=\"8e099d0\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"font-size: 1em; background-color: transparent;\">Kognitive Verzerrungen werden mit Computer-basierten Programmen behandelt \u2013 den Cognitive-bias-modification(CBM)-Programmen. Randomisierte kontrollierte Studien, in denen MBSR mit aktiven Kontrollbedingungen verglichen wird, deuten darauf hin, dass MBSR bei der Verringerung der Schwere von Angst- und Depressionssymptomen bei Personen mit einem breiten Spektrum medizinischer und psychiatrischer Erkrankungen moderat bis hoch wirksam ist.<\/span><sup style=\"color: #171717; font-family: 'Source Sans Pro', sans-serif; font-weight: 400; background-color: transparent;\">4<\/sup><\/h4><p>Die bisher umfassendste \u00dcbersichtsarbeit untersuchte die Auswirkungen von 209 Studien zu achtsamkeitsbasierten Interventionen bei 12.145 Patienten mit einer Vielzahl von St\u00f6rungen, inklusive Depressionen. Die Ergebnisse zeigen \u00dcberlegenheit zu Warteliste, Psychoedukation, unterst\u00fctzende Psychotherapie, Entspannungs\u00fcbungen und Imaginations- und Unterdr\u00fcckungstechniken. Die Effektst\u00e4rke war f\u00fcr psychologische Parameter bei \u00c4ngsten gefolgt von Depressionen am gr\u00f6\u00dften<sup>.4<\/sup><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e8f7dc4 elementor-widget elementor-widget-heading\" data-id=\"e8f7dc4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Mindfulness-based Cognitive Therapy (MBCT)<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-41330b3 blog-text elementor-widget elementor-widget-text-editor\" data-id=\"41330b3\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h4>Mindfulness-Based Stress Reduction (MBSR)<\/h4><p>Le MBSR est un programme de 8 semaines bas\u00e9 sur des exercices r\u00e9guliers de m\u00e9ditation et de pleine conscience, ainsi que sur le yoga, des exercices de respiration et des \u00e9changes en groupe. Des \u00e9tudes contr\u00f4l\u00e9es randomis\u00e9es comparant le MBSR \u00e0 des conditions de contr\u00f4le actives indiquent une efficacit\u00e9 mod\u00e9r\u00e9e \u00e0 \u00e9lev\u00e9e dans la r\u00e9duction de la s\u00e9v\u00e9rit\u00e9 des sympt\u00f4mes d\u2019anxi\u00e9t\u00e9 et de d\u00e9pression chez des personnes pr\u00e9sentant un large \u00e9ventail de troubles m\u00e9dicaux et psychiatriques.<sup>4<\/sup> <\/p><p>La m\u00e9ta-analyse la plus compl\u00e8te \u00e0 ce jour a examin\u00e9 209 \u00e9tudes portant sur des interventions bas\u00e9es sur la pleine conscience aupr\u00e8s de 12 145 patient\u00b7e\u00b7s pr\u00e9sentant divers troubles, dont la d\u00e9pression. Les r\u00e9sultats montrent une sup\u00e9riorit\u00e9 par rapport aux listes d\u2019attente, \u00e0 la psycho\u00e9ducation, \u00e0 la psychoth\u00e9rapie de soutien, aux exercices de relaxation ainsi qu\u2019aux techniques d\u2019imagerie et de suppression. L\u2019ampleur de l\u2019effet \u00e9tait la plus importante pour les param\u00e8tres psychologiques li\u00e9s \u00e0 l\u2019anxi\u00e9t\u00e9, suivis de la d\u00e9pression.<sup>4<\/sup>  <\/p><h4>Mindfulness-based Cognitive Therapy (MBCT)<\/h4><p>Le MBCT est structurellement et temporellement similaire au MBSR. Il combine des \u00e9l\u00e9ments d\u2019entra\u00eenement \u00e0 la pleine conscience et de th\u00e9rapie cognitive afin de pr\u00e9venir les rechutes d\u00e9pressives. Les variations de l\u2019humeur sont d\u2019abord observ\u00e9es sans jugement, puis associ\u00e9es \u00e0 de nouvelles fa\u00e7ons de penser.<sup>5<\/sup>  <\/p><p>Plusieurs \u00e9tudes contr\u00f4l\u00e9es randomis\u00e9es d\u00e9montrent une r\u00e9duction du taux de rechute, en particulier chez les patient\u00b7e\u00b7s \u00e0 haut risque de rechute (Cohen\u2019s h = 0,88).<sup>5<\/sup><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div data-dce-title-color=\"#206A79\" class=\"elementor-element elementor-element-cc16a3a elementor-widget elementor-widget-heading\" data-id=\"cc16a3a\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">CBM et d\u00e9pression <\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-512887e elementor-widget elementor-widget-heading\" data-id=\"512887e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Aufmerksamkeitstraining f\u00fcr positive Reize<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f19c078 elementor-widget elementor-widget-text-editor\" data-id=\"f19c078\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>La CBM cible l\u2019attention port\u00e9e aux stimuli positifs ainsi que l\u2019interpr\u00e9tation des exp\u00e9riences. Beevers et al. (2015) ont montr\u00e9, dans une \u00e9tude de 4 semaines men\u00e9e aupr\u00e8s de patient\u00b7e\u00b7s adultes, qu\u2019un entra\u00eenement \u00e0 l\u2019orientation attentionnelle vers des stimuli positifs entra\u00eenait une r\u00e9duction significative du biais attentionnel n\u00e9gatif, associ\u00e9e \u00e0 une diminution significative des sympt\u00f4mes d\u00e9pressifs.<sup>2,6 <\/sup>Des r\u00e9sultats comparables ont \u00e9t\u00e9 observ\u00e9s chez des patient\u00b7e\u00b7s d\u00e9pressifs en r\u00e9mission ainsi que chez des adolescents.<sup>2<\/sup>  <\/p><p>Cristea et ses coll\u00e8gues concluent que la CBM, en tant qu\u2019entra\u00eenement attentionnel et interpr\u00e9tatif, montre une am\u00e9lioration significative.<sup>7<\/sup> Elle peut donc \u00eatre utile lors d\u2019\u00e9pisodes l\u00e9gers \u00e0 mod\u00e9r\u00e9s, dans la pr\u00e9vention des rechutes et en compl\u00e9ment d\u2019autres approches dans les cas de d\u00e9pression s\u00e9v\u00e8re.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div data-dce-title-color=\"#206A79\" class=\"elementor-element elementor-element-b9f00e8 elementor-widget elementor-widget-heading\" data-id=\"b9f00e8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">R\u00e9alit\u00e9 virtuelle (VR) contre la d\u00e9pression bas\u00e9e sur MBI et CBM <\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-9fa7929 elementor-widget elementor-widget-text-editor\" data-id=\"9fa7929\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Les donn\u00e9es scientifiques issues de plusieurs \u00e9tudes randomis\u00e9es et contr\u00f4l\u00e9es ind\u00e9pendantes soutiennent l\u2019utilisation d\u2019entra\u00eenements \u00e0 la pleine conscience et \u00e0 l\u2019attention dans le traitement des d\u00e9pressions, y compris s\u00e9v\u00e8res, en compl\u00e9ment d\u2019autres approches telles que les psychopharmaceutiques et la th\u00e9rapie comportementale. On peut s\u2019attendre \u00e0 une r\u00e9duction plus marqu\u00e9e des sympt\u00f4mes, \u00e0 une augmentation du taux de r\u00e9mission ainsi qu\u2019\u00e0 une am\u00e9lioration de la pr\u00e9vention des rechutes. <\/p><p>Des applications int\u00e9grant des MBI et des programmes CBM assist\u00e9s par ordinateur sont d\u00e9j\u00e0 utilis\u00e9es.
